flood of email notifications indicating access revoked

One of our users received a flood of emails the night of October 1 around 8pm indicating his access had been revoked to one of our private repos. a) we could not determine who (if anyone) revoked his access, and b) 123 emails doesn't right. Could you guys take a look and give us some info on what happened? Thanks!
