Create Branch dialog shows duplicate forms after switching between tabs

In the new UI of Bitbucket, click create a new branch the dialog had overlapping forms and Branch From field was stuck at label "Loading"

Steps to produce the bug:

1. Go to Branches tab. by default Active tab is selected.
2. Click on Merged tab.
3. Click on Active tab.
4. Click on Create Branch.
