Creating a new branch - duplicate/broken form

Go to https://bitbucket.org/<org>/<project>/branches/?status=active
and swap to 'merged' filter, and back to 'active'.

Now click on 'Create Branch' and notice the form is broken, with duplicate dropdowns.
