Pull Request merged unknowingly with my account, I didn't merge it.

I have not merged a pull request in my organization project but It is showing that I have merged it. Also, my teammate got mail about it but I didn't receive any mail about it.

I don't know how this happened, But It's a serious issue with my account. I want to know more details about it.
