Branch permissions user interface is not working correctly.

As mentioned in the title, attempting to add branch permission through the user interface is not working correctly. The "Write access" and "merge via pull request" textboxes are not editable after adding a pattern or branch name on the "Branch or pattern" textbox

Steps to reproduce

1. Navigate to the repository > Settings > Branch permissions
2. Create a new branch permission
3. Type anything on the "Branch or Pattern" textbox
4. It will not be possible to edit either the "Write access" and "merge via pull request" textboxes, only to select from the dropdown that shows up
