TOC macro requires balanced equal signs in headers

The TOC macro requires balanced equal signs in the heading, but this is not required by the Creole 1.0 syntax spec. The heading is rendered properly in the page, but it does not show up in the TOC.

This is not a big problem - if you know it. If you don't know it then you are wondering what you are doing wrong. The first place you'll look is at how you use the {{{<<toc>}}} macro. Since that is the wrong place to look, it gets frustrating quickly.

Because the heading renders properly in the wiki page, you assume your markup can't be the problem.
