Access revoked notification doesn't say which fork it applies to (BB-3804)

I received a notification that my access rights to a public repo were revoked. However, when I investigated, this was for a //fork// of the original repo that I had access to, but the message didn't say that. This is very confusing. The message text should say,

You have received a notification from xxxxx.\\\

You no longer have access to the source of xxxxx/repo.

Currently, it has just repo instead of xxxxx/repo.
