Create Team Page not working

The create a team page (https://bitbucket.org/account/create-team/) is not allowing me to create a team. Whenever I fill out all of the fields and click "Create", it redirects back to the same page. It doesn't display any errors to let me know if I've done something wrong (all fields are filled out, so I don't know what the problem is).
