Previous approvals should be unapproved when a Pull Request is updated and not yet merged

An initial pull request can be submitted by Bob. Teammates review and approve. Bob decides to update his pull request with subsequent commits introducing errors. Previous approvals still stand as misnomers when the PR has changed.

This is a problem and not accurate leading to merges of potentially problematic code.

Request to remove previous approvals if a PR is updated before it is merged.
