Hook with no configuration (e.g. Issues) lost in space... (BB-10826)

When you add a Hook without any configuration, e.g. Issues, it gets lost in space if you do the following:

1. Select Issues from the dropdown
2. Click Add
3. Click Edit on a previous added Hook, e.g. Email
4. Click Cancel in the dialog that pops up...

...now the added Issues hook takes a trip...

If you click Save in the dialog that pops up, the added Issues hook stays, or if you navigate away from the Hooks view and then return it also stays...
