Modified service to use the TopShelf libraries so it can easily be run as a service or console, as well as easily install / uninstall multiple instances, also added a "prefix" attribute to the configuration so you can override the "Bson" naming convention for all of your document tables and index tables - make it an empty string to not use any prefix

added new index/drop command, allow you to specify an index key with ExtractOnly - which makes a column for the value but does not included it in the actual index...allows you to query on it but not include it in Sql's index
