Review design of CrossfilterDashboard for individual entities

This is an overall refinement of individual schools dashboard:

• adding dashboard child with teachers data inside individual schools

• adding dashboard child of flow indicators based on outcome warehouse data (currently flows are based on reconstructed cohort and raise too many questions/concerns) for individual schools (this should also apply to the Indicators dashboard as required by #362)

• Combine some of the redundant dashboard components (i.e. chart and table combined and available on blown out version as seen other places)
