Exams upload issue

There seems to be a bug in FedEMIS. When we upload the exam file we get.

Furthermore, they seem to get duplicated in the system.

    This now seems to work with latest SQL and app upgrade. I also cleaned up all previous exams data to start from scratch in FSM.
