lkpExamTypes support for sorting

It would be nice to have a new column to code in the desired sort order of the exams. I think best place for this is in lkpExamTypes. Then the sorting in reports could be configurable through the lookups without having to hard code by countries.

Once this is done. Create related issues for the Exams JasperReports making the sorting simpler and generic.
