Change hierarchy of reports

So that only entities reports are displayed into entities->reports and only entity reports are displayed under an entity's reports tab.

Principally this means making Teachers and Teachers/Teacher folders to be siblings, rather than parent child; and adjusting the route definitions for Teacher reports accordingly.

This was done. So before deploying to other countries next time this should be taken into consideration. I will take care of FSM/RMI but I don't know how the reports for SI and KI are managed.
