Closing this, but noting it would be better to have a lower security account, and preferably Windows Authentication.

This is a note about what needed to be done to get this to work:
On the Exchange Server:
we need a receive connector to relay from the IIS server:

and the IIS Server has to forward mail to the exchange server:
Currently we have got his working by specifying the credentials of a user with an exchange mailbox.
It would be best if this can be the same account running the apppool - this account also needs access to the SQL server, so it centralises security setup in this one place.
Only made it work with a Domain Admins - so more to do to figure out what is the minimal security requirement.
