Report teacher validations in Workbook upload prior to processing

In a validation stage before loading, report errors on the teacher page that will cause upload problems.

These include:

-- duplicate teacher records ( either at the same school or different schools

-- unknown code for teacher qualifications

-- unknown codes for teacher roles
