Adding dirfix to multiple releases as regular user fails

One dirfix can apply to multiple releases. We add the dirfix to all those releases. A regular user uploads it to the first release and the dirfix gets added to the unconfirmed queue. All the next attempts will fail, but a message such as "Successfully suggested file Power.2014.S01.DIRFIX.720p.BluRay.x264-DEMAND/power.2014.s01.dirfix.720p.bluray.x264-demand.nfo" is shown.

It still worked on 2013-10-29.

Only file name, file size and crc32 were taken into account to filter dupes. Release name added to the dupe check.
