Add a guide about connecting to cloud-based TFS

Configuration steps are similar to using Visual Studio with cloud-based TFS services. I'll add the detailed instructions to the wiki. Meanwhile you can refer to:

for codeplex: http://lajak.wordpress.com/2011/02/10/how-to-use-team-explorer-with-codeplex/ (the main gotcha is that username is prefixed with snd\ and suffixed with _cp, also you need to create a codeplex user password)
