[TIME-253] Timesheet plugin under JIRA v5.0.1 does not render column widths properly, and displays the Project and Issue columns twice.

We recently upgraded to JIRA v5.0.1 and the corresponding latest version of the Timesheet plugin. Now when we generate a Timesheet report, the issue Summary column consumes most of the available space, and the Comment column is very narrow (see the attached screen shot). This seems to be true under Firefox and IE.
