Hi!
1. Jira Service Desk support only built in customfield types and not supported third party customfield types (see https://jira.atlassian.com/browse/JSDSERVER-86, https://jira.atlassian.com/browse/JSDSERVER-787, https://jira.atlassian.com/browse/JSDCLOUD-787)
2. This app only for server version. I do not have resources for make cloud version
