Support http basic auth for apache-status

Allow testing an apache-status protected by http basic auth, e.g.

 if failed port 80 protocol apache-status 
   path /status
   http-user admin
   http-password 1234
        loglimit > 10% or 
        dnslimit > 50% or
        waitlimit < 20%
 then alert

i.e. add http-user and http-password options

Syntax:

if failed port 80 protocol apache-status
    username myuser
    password 123456
    loglimit > 10% or
    dnslimit > 50% or
    waitlimit < 20%
then alert
