Start run dialog text field size should be same as "edit on start" odb entry size

Run start dialog allows test field much longer than the underlying odb entry, i.e. for "run comment", with result that text typed by user is truncated. Start.html web page should create text entry field of the same length as the ODB variable - from the db_ls json field "item_size". K.O.
