The button is displayed on the first visit to each page after purging the Moodle caches. On subsequent visits to the same page the button no longer shows. No console errors in Chrome or Firefox, and the debug logs aren't printed after the first page visit either.

Looks like this is being caused by changes, that mean that the lib.php file in local plugins is no longer loaded on every page request.
Caused by a combination of two Moodle changes:
It starts working when I add a dummy navigation extension callback to lib.php.

Should be able to fix it using https://docs.moodle.org/dev/Automatic_class_loading.

This should now be fixed in the current master branch. (it is not up in the plugins directory yet)

Ctrl+Alt+B also doesn't work after the first visit.
