OP List shows PLAYER_NAME_UNKNOWN for some operators

Operator List fails to convert a UUID to a given player's name instead displaying PLAYER_NAME_UNKNOWN

At the very least it should at least display an UUID failsafe for verification purposes

changed OpList to display the UUID of a unknown name operator resolves #1 add method to DamageSource to get is it is indirect or not
