I'm getting a 404 on the ajax call to add a comment

1. Open an existing Issue
2. Comment on the issue normally (this works)
3. Click the Reply link below the comment you just entered
4. Enter comment text and click Add
5. Get 404 in browser console:

I really didn't manage to look into this problem. Please go ahead and try, it should work.

All these problems should be resolved now with the latest release 1.12. Please check it and if you find any issue, please raise a new request per problem.
