Sermon Notes Upload not working

When uploading a pdf as notes file for a sermon.

File uploads but when you hit button to associate file as sermon notes file, the media dialogue goes blank and you get a js error.

There is the following js error:

Cannot call method 'replace' of undefined

File appears as attachment on single sermon page but not on archive. This has recently stopped working as we have a number of sermons successfully created with notes files.

the error is being generated inside the meta box code you are using.

I can't reproduce this. What theme and other plugins are you using?

All plugins disabled (exc SM) and twenty twelve theme - broken.

On a totally fresh WP install with only Sermon Manager on my PC - all ok!

Console still sees bug in cmb.js line 74.
