Add support for building Windows CHM files from the documentation

I find windows help files for a module/framework very helpful to have around when developing s/w on top of that framework. There are many reasons but I suspect speed of search and navigation are the most important.
I would like to add support to the doc/build/Makefile for a htmlhelp target. Please find attached an incomplete patch: stuff to be figured out is how to run hhc.exe (the help compiler executable on win32).

Seems to be in the Makefile already, probably when we upgraded Sphinx awhile back.
