Show plugin name with description of instruction items (the tooltip)

There are getting to be a LOT of plugins and plugin instructions, and many of these are intermixed in existing categories. When looking at the instruction sidebar, then, it’s hard to know where each of these instructions comes from (built in, or from a particular plugin).

My suggestion is that the tooltip for instructions in the sidebar include the plugin name, if any.
