Clustering fails with message "Measuring distances encountered an error"

While testing a new matrix file (attached), we were unable to cluster it (with all the default settings) because an error kept popping out "Measuring distances encountered an error". The matrix has NaNs (indicating missing data).

I am going to look into this once I am done with getting the data loading right. This is pretty major so I'll give it priority.

resolves #250; commented out some old cluster code because it's not clear what it does and it caused significant unacceptable problems; renamed CDTGenerator to ClusterFileGenerator

Checked on version 20151118, issue fixed (used the same matrix file for testing).
