Spot removal looks weird

Hey, I am trying to use the spot removal tool, but the results look broken. It is a pixelated square with a lot of noise that does not look like the source. I attached images below to show the problem.

The issue gets worse the darker the image is. On very bright images with lots of light, it does not appear.
