Inconsistency Queue Error

happening after talking to barathrum after brightsheol. Leaves a copy of the player character in the tile they would have entered the map in and the active player character somewhere else on the map. Also, the respawn at nyd- +3 ego dismembered faces did not carry over into the chest, perhaps it was because they are a corpse category item?

The order of events was, talk to barathrum after sheols, save and quit, load, first instance of inconsistency queue error- copy standing beside barathrum, recoil to stilt to hand in reshelph secrets and books for xp, leaving the stilt, happens again in screenshot.
