"add URL" button on dialog at /publication/USER/HASH not at correct position

When I want to add another URL to a publication, a nice new dialog appears. Unfortunately, the "add URL" button is not at the same vertical position as the "close" button.

I would also suggest to omit the "close" button, since the dialog has a closing symbol at the upper right corner. Having the close button at the lower right corner might also be misleading (I think there are many discussions, if "save" should be left and "close" right or the other way around.)

BTW: when I finally add the URL, the AJAX request does not work - the browser goes to the URL http://bootstrap3.biblicious.org/ajax/additionalURLs and shows me the JSON response.
