Installed 3.1.4-BETA, still can't see any options dialog.

Max, could you try uninstalling and then installing ? I think I have an issue where installer for some reasons does not overwrite DLLs that are already in place..

Do I need to close SSMS as well before uninstalling?

Installation didn't really work until I closed SSMS and uninstall the plugin first.

Filed another ticket: Bug ~~#72~~.
