Add custom profile field check to alerts

We need to be able to target alerts to custom profile fields.

1. Add setting to choose how each alert will display (global, users, admin and profile field)
2. Add profile fields to settings page (field name / expected value)
3. Move code from header.php to renderer where functions can be used to evaluate conditions and return alert code
