The manual http://www.pp.rhul.ac.uk/bdsim/manual/model_description.html#samplers-output says this:
Warning A sampler attached to the first item (therefore at the beginning of the beamline) may not record all primary particles. This is due to the bunch distribution having a finite length in z and some of the particles (typically half) start in front of the sampler. This is not an error, but as expected. It is best not to put a sampler on the first element, but to use the recorded primary coordinates in the output.
But I think you are right that perhaps we shouldn't put a sampler at the very first element for the all samplers. To be discussed.
