Un-escaped characters in HTML filter

It seems the <pre> elements are not rewritten properly with the HTML filter. For example something like <code><b></code> inside <pre> will be output <code></code> causing bolding afterward. See the attached example where the bottom of the .out file is de-formatted.

There are still cases (in <code> inside <p> for example) where the character < does not get escaped properly. See r1253 for a snippet.

the phrasing of the bug confused me a bit. After thinking about it this has nothing to do with pre or code tags. The Html encoder was not being run as it should be. I know set the mime type on each TextUnit and all test cases pass.
