SimpleTM leveraging: Unapproved.tmx entries have source not target in target element.

When preparing for RTF+Original, any unapproved translation generated in
the unapproved.tmx file have for translation the source text instead of
the target.

Actually this happens only when SimpleTM is on. And it occurs in all outputs, not just RTF.
