CT and DX histogram-filtered acquisition data don't respect all specified filters

CT and DX histogram-filtered data only respect the acquisition protocol, min and max DLP fields. They need to adhere to the others, such as manufacturer, study date etc.

I've fixed the acquisition histogram data for Dx (d570add) and CT (4f14621).

The CT study histogram doesn't respect the additional filters either - need to fix that.

Histogram-filtered DX and CT data now respects all filters for acquisition and study plots (resolves issue #143 and resolves issue #139).
