New userprofile database fields are not in the migration files

Attempted an upgrade from 0.6 to latest develop, and wasn't able to see the homepage due to the error column remapp_userprofile.plotDXStudyMeanDAP does not exist

This field and other user_profile related fields that have recently been added to models.py haven't been added to the migration files.

The same is true for some of the DICOM fields, but the impact is reduced. I'll fix that afterwards.

Turns out there was only one table I needed to change. I'll test this later with a view to closing.

Used to upgrade a 0.6 database to 0.7. All problems seem to have been resolved.
