Some Requested Procedure Code Sequences are longer than allowed 16 characters

And thus fail to import.

The standard dictates the VR of the CodeValue is SH, which is limited to 16 characters. It seems that Siemens permits longer strings to be used.

Propose changing the datatype in the database to relax this constraint.

Changed code_value fields from SH/16 char fields to TextFields to ref #501. Tested with SQLite3, need to test with PostgreSQL
