Handle more bad input in PDF Survey load

a number of problems emerge when the PDF urvey is completed in Chroma, rather than the intended Acrobat DC:

1. File grows inordinately large
2. Data type is ignored on inputs e.g. text can be entered in numeric fields
3. in code driven dropdwons, the description is provided as the field value, not the code
4. totals on grids - which are clculated in Javascript in the pdf - are not displayed. (Note these totals are not moved to the database, but are calculated in the DB from the grid values)

These revision to Teacher loading work around some of these problems, to load pme exisitng forms . But for the future, ussers must complete the form using Acrobat DC
