Enhancements to lookup editing

• Reinstate the input-grid styles ( used pre-materialize)
• Insert spaces into table name in dropdown list for better readabaility
• Allow navigation directly to a table from url - populate the table list with the correct item
• retrieve and display any [Description] attribute on the entity object
• support dropdowns populated by an array as well as from a lookup table
• insert a null entry into dropdown lists populated form a lookup , to allow the setting to be cleared.

All part of getting a better result on editing of CostCentres #702
