Support 'report' formats in warehouse accreditation views.

In enrolment warehouse views, ‘report’ format denormalises on gender, providing 2 columns - M and in the output, rather than a disaggregating ‘Gender’ field in each row.

Analogously, ‘report’ format in the accreditations view will provide 4 columns Level1, Level2, Level3, Level4 rather than a disaggregation on ResultLevel. This will apply to ‘accreditations’ view (final level of the inspection) and ‘byStandard’ views.
