OpenXML: PPTX corruption when using formulas (m: prefix)

The attached testcase uses an embedded formula from the http://schemas.openxmlformats.org/officeDocument/2006/math namespace, which breaks the filter (and corrupts the document) during a roundtrip.

    As of commit 153a6ce this no longer corrupts the document entirely, but it does mangle some portions of the formula itself. It looks like consolidation of runs in the math namespace (<m:r>) may be the culprit.
