Wrong highlight for new index entries

As a side effect of #206, there is now a problem with adding a new entry to the index: multiple redundant highlights are added to the URL.

    Before adding an entry all existing highlights should be (temporarily?) cleared. Looks like the "h (hide) command doesn't clear them, but actually hides them: they still exist but are not displayed. At present the new coordinates are inserted into all the highlights.

    To reproduce just add an entry to the enclosed index.
